What happens first?
Begin with a private intake. After you submit it, you may stop or continue to payment, consent, and appointment selection.
Is the first visit in person?
Yes. New and transfer evaluations take place at our Pompano Beach office. Renewal visits are available by telehealth.
What does the evaluation cost?
A new evaluation is $175. Renewals and transfer evaluations are $125. Florida's state application fee is separate.
Does payment guarantee certification?
No. Payment covers Dr. Kim's evaluation. Certification depends on the medical evaluation and Florida requirements.
What should I bring?
Bring photo identification, a current medication list, and any records that may help Dr. Kim understand your history.
What if I am not sure my condition qualifies?
You do not need to interpret the law before starting. Tell us what you would like help with, and Dr. Kim will review it with you.
