First-Time MMJ Evaluation

Florida requires the first MMJ certification evaluation to be completed in person.

Confirm your appointment

Payment is for the physician evaluation, medical review, required consent review, and registry-related work if certification is clinically appropriate. Certification is not guaranteed, and the Florida state MMUR card/application fee is separate.
