Getting Anxiety Medication via Telehealth โ What's Possible
Short answer: yes, most anxiety medications can be prescribed through telehealth. The longer answer involves understanding which medications, what the process looks like, and where the limitations are.
If you're dealing with anxiety and want treatment without sitting in a waiting room (which, ironically, makes anxiety worse), telehealth is a legitimate path to getting help.
Medications That CAN Be Prescribed via Telehealth
SSRIs (first-line treatment for anxiety):
- Sertraline (Zoloft)
- Escitalopram (Lexapro)
- Fluoxetine (Prozac)
- Paroxetine (Paxil)
These are non-controlled, can be prescribed at any telehealth visit, and are considered the gold standard for generalized anxiety disorder, social anxiety, and panic disorder.
SNRIs:
- Venlafaxine (Effexor)
- Duloxetine (Cymbalta)
Also non-controlled, effective for anxiety, and often used when SSRIs aren't enough or when anxiety co-occurs with pain.
Buspirone:
- Non-controlled
- Works specifically for anxiety (not depression)
- No addiction risk
- Can be prescribed freely via telehealth
Hydroxyzine:
- Non-controlled antihistamine
- Works quickly for acute anxiety
- Non-addictive
- Commonly prescribed via telehealth as a PRN (as-needed) option
Propranolol:
- Beta-blocker, non-controlled
- Excellent for performance anxiety, physical symptoms of anxiety
- Frequently prescribed via telehealth
Gabapentin:
- Schedule V in some states (not federally scheduled)
- Used off-label for anxiety
- Generally prescribable via telehealth
Medications That Have Telehealth Restrictions
Benzodiazepines (Xanax, Ativan, Klonopin, Valium):
- Schedule IV controlled substances
- CAN be prescribed via telehealth in Florida with an established patient relationship
- Many telehealth providers choose not to prescribe them as first-line
- If you're already on a benzodiazepine from another provider, continuing it via telehealth is generally straightforward
- New prescriptions for benzodiazepines may require documentation of failed alternatives
Why the caution with benzodiazepines: They work fast (which patients like) but carry dependence risk, aren't meant for long-term use, and have significant interaction risks. A responsible doctor โ telehealth or in-person โ will discuss whether they're truly the best option for your situation.
The Telehealth Anxiety Evaluation
Here's what to expect during a telehealth visit for anxiety:
Before the visit:
- Complete intake forms about symptoms, duration, severity
- List current medications and past treatments
- Note any specific triggers or patterns
During the visit (20โ30 minutes):
- Your doctor assesses symptom severity (often using validated scales like the GAD-7)
- Reviews your medical and psychiatric history
- Discusses previous treatments โ what worked, what didn't
- Rules out medical causes of anxiety (thyroid, hormones, medication side effects)
- Recommends a treatment plan
After the visit:
- Prescription sent to your pharmacy
- Follow-up scheduled (usually 2โ4 weeks for new medications)
- Instructions on what to expect as medication takes effect
What Telehealth Doctors Look For
A thorough anxiety evaluation includes:
- Duration: How long have you been experiencing symptoms?
- Severity: Is anxiety interfering with work, relationships, sleep?
- Physical symptoms: Heart racing, chest tightness, sweating, GI issues?
- Previous treatment: Have you tried therapy, other medications?
- Medical history: Thyroid issues? Stimulant use? Other medications that could cause anxiety?
- Substance use: Caffeine, alcohol, cannabis โ all affect anxiety
- Suicidal ideation screening: Standard for any mental health evaluation
This evaluation works perfectly well via video. Anxiety diagnosis is primarily based on reported symptoms and clinical history, not physical examination.
Timeline: When Will Medication Start Working?
This is critical to understand:
- SSRIs/SNRIs: 2โ6 weeks for full effect. Many patients feel worse the first week.
- Buspirone: 2โ4 weeks for full effect
- Hydroxyzine/propranolol: Works within 30โ60 minutes (used as-needed)
- Benzodiazepines: Work within 15โ30 minutes (but this is why they're addictive)
A good doctor will:
- Start you on a first-line medication (usually SSRI)
- Give you something for acute episodes while waiting for the SSRI to work (hydroxyzine or propranolol)
- Follow up in 2โ4 weeks to assess response
- Adjust as needed
Common Concerns About Telehealth for Anxiety
"Will the doctor take me seriously?"
Yes. Anxiety is the most common mental health condition. Any physician experienced in mental health recognizes it and treats it routinely.
"Can they tell I'm anxious over video?"
Often, yes. But more importantly, they'll listen to what you describe. Anxiety diagnosis doesn't require physical examination.
"What if I need therapy too?"
Many telehealth practices offer both medication management and therapy referrals. Medication + therapy is the most effective approach for most anxiety disorders.
"What if the first medication doesn't work?"
That's normal. It takes trial and error for many patients. Your follow-up visits are where adjustments happen. This is actually easier via telehealth โ you're more likely to keep follow-up appointments when they're from your couch.
When Telehealth for Anxiety Isn't Appropriate
- Active suicidal crisis (go to ER or call 988)
- Severe panic attacks causing inability to communicate
- Suspected anxiety caused by acute medical emergency (chest pain could be cardiac)
- Need for inpatient psychiatric care
For the vast majority of anxiety โ generalized worry, social anxiety, panic disorder, health anxiety โ telehealth is perfectly appropriate and often preferred by patients who find medical settings triggering.
